What Is Neck Pain?

Neck pain is pain or stiffness in the cervical spine, the seven vertebrae, discs, facet joints, nerve roots and supporting soft tissues that carry the head. It is the second most common musculoskeletal complaint after lower back pain. The neck supports a head weighing about 5–6 kg while allowing the widest range of motion anywhere in the spine, and that combination makes it vulnerable, particularly with long hours of phone and desk use, often called 'tech neck'. Symptoms range from morning stiffness that clears within days to persistent pain spreading into the shoulder, arm and hand, which suggests a nerve root is involved. At Bewell's Novena clinic, opening soon, a Gonstead chiropractor assesses each neck segment individually, by hand: observing your posture, checking for temperature differences along the spine with an instrument, and feeling how each level sits and moves. Only a segment the findings point to is adjusted. That specificity matters in the neck, where any force needs to be applied with care. Neck pain after a fall or accident, or with a severe headache, dizziness, clumsy hands, trouble with balance or worsening arm weakness, should be checked by a registered doctor first.

Root Causes

What Causes Neck Pain?

Neck pain almost always has a mechanical origin: a specific structure under ongoing stress or sudden strain. These are some of the most common contributors.

Forward Head Posture (Tech Neck)

The further the head moves forward from neutral, the more load the neck has to carry. Hours of looking down at phones and laptops are a common contributor to neck pain in adults under 40.

Prolonged Desk Work

Sustained static postures at a workstation — monitor too low, chair too high, or keyboard too far — create chronic muscle fatigue and uneven loading on cervical discs and facet joints throughout the working day.

Cervical Disc Degeneration

Discs between cervical vertebrae lose hydration and height with age, reducing shock absorption and narrowing the foramina where nerve roots exit the spine. Degenerated cervical discs are a primary source of arm pain and tingling.

Whiplash & Injury

Sudden acceleration-deceleration forces — car accidents, sports impacts, falls — strain the soft tissues and joints of the neck. Symptoms often emerge days after the event, once inflammation sets in.

Poor Sleep Position

A pillow that is too flat, too high, or too soft keeps the cervical spine in a stressed position for 6–8 hours every night. Over time, this can add to ongoing neck strain and morning stiffness.

Stress & Muscle Tension

Psychological stress causes the trapezius and levator scapulae muscles to tighten chronically. Sustained muscular tension pulls on cervical vertebrae over time, gradually disrupting spinal alignment.

Progression

How Neck Pain Progresses

Neck pain can change over time, from simple stiffness to nerve involvement. Knowing the stages helps you recognise when to get checked, and when to see a doctor.

Stage 1Mild

Acute Stiffness & Muscle Pain

Localised pain and restricted rotation, often triggered by a specific incident or an awkward sleeping position. Muscle guarding limits movement. At this stage the problem is mostly in the soft tissues, and many episodes settle within days to weeks.

Stage 2Moderate

Radiating Pain & Nerve Involvement

As disc pressure or joint irritation increases, pain begins to radiate into the shoulder, upper arm or forearm. Tingling or numbness in the fingers may appear; this is cervical radiculopathy. The nerve root is now being compressed, not just irritated.

Stage 3Severe

Chronic Pain & Postural Change

Persistent low-grade pain with visible changes: forward head carriage, rounded shoulders and reduced range of motion. Muscle imbalances become entrenched. Left unexamined, the neck tends to adapt around the problem, which can make care more involved.

Stage 4Critical

Cervical Myelopathy

In advanced cases, degenerative changes or disc material press on the spinal cord itself, not just a nerve root. Signs include weakness or clumsiness in the hands, balance problems and difficulty with fine motor tasks. This needs urgent assessment by a registered doctor.

Most neck pain is mechanical.

A Gonstead assessment looks at each cervical segment individually, so any adjustment is specific rather than general.

Common questions

Gonstead neck adjustments are specific and controlled, and are only done after a proper examination. If adjustment is not suitable for you, we will tell you and suggest you see a doctor.

Recurring neck pain often means a joint that is not moving well, posture strain or a daily habit is still adding load to the neck. A Gonstead assessment looks at how your neck and the rest of your spine are actually moving, not only where it hurts.

Neck problems can contribute to headaches, especially when the upper neck is involved. Dizziness always needs careful assessment, so please see a registered doctor first to rule out other causes.

It is best avoided. You may be moving joints that are not the real problem, and the brief relief can, over time, add to irritation and looseness in those joints.
