What Is Back Pain?

Back pain is pain or stiffness felt along the spine, most often in the lower (lumbar) back, and it usually comes from the joints, discs, muscles, ligaments or nerves of the back. It is one of the most common health complaints worldwide, affecting most adults at some point in their lives, and ranges from a dull, persistent ache to a sudden, sharp pain that makes moving difficult. The lumbar spine bears most of the body's weight and is the most frequently affected region, though mid-back and neck pain are also widespread. Back pain often keeps returning when the mechanical contributors, such as posture, sitting habits or how particular spinal joints move, are never looked at. At Bewell's Novena clinic, opening soon, a Gonstead chiropractor assesses your spine and pelvis by hand, segment by segment: observing your posture, checking for temperature differences along the spine with an instrument, and feeling how each level sits and moves. Adjustments are made by hand, only at the segments the findings point to. What Causes Back Pain?

Back pain is almost always mechanical: something in the spinal structure is not working the way it should. Understanding which structure is involved helps shape the care that suits you.

Muscle Strain & Ligament Sprain

Sudden awkward movements, heavy lifting, or repetitive strain can overstretch muscles and ligaments in the back. This is the most common cause of acute back pain, often felt as a sudden 'catch' in the lower back.

Poor Posture & Prolonged Sitting

Sustained forward-flexed postures place uneven load on spinal discs and facet joints. Years of desk work, driving, or looking down at phones accelerates spinal wear and shifts vertebral alignment.

Disc Degeneration & Bulging

Worn or herniated discs reduce the cushioning between vertebrae and can compress nearby nerves. Disc problems are responsible for a significant proportion of persistent, radiating back pain cases.

Facet Joint Dysfunction

The small facet joints linking each vertebra can become inflamed or restricted, causing localised pain that worsens with twisting or bending backward. Often felt as stiffness after rest.

Spinal Subluxation

A spinal segment that is not sitting or moving as it should, which chiropractors call a subluxation, can change how load is spread across the spine and irritate nearby tissues. Finding these segments is the focus of a Gonstead assessment.

Sedentary Lifestyle & Weak Core

Weak abdominal and back muscles provide insufficient support for the spine. Without adequate muscular support, the vertebral structures absorb forces they are not designed to handle alone.

Progression

How Back Pain Progresses

Back pain often follows a recognisable course. Knowing the stages helps you decide when it is worth having your spine assessed.

Stage 1Mild

Acute Pain (0–6 Weeks)

Sudden onset, often after a specific incident such as lifting or twisting. Pain is sharp or intense and may limit movement significantly. Inflammation is high. Many acute episodes settle within a few weeks, and an assessment at this stage helps you understand what set it off.

Stage 2Moderate

Subacute Pain (6–12 Weeks)

Pain fluctuates but lasts beyond the few weeks an acute strain usually takes to settle. The body begins adapting, developing protective muscle spasms and altered movement patterns that can create secondary pain elsewhere. The mechanical contributors are still there.

Stage 3Severe

Chronic Pain (3+ Months)

Persistent pain that has become part of daily life. Structural changes such as disc degeneration, facet joint wear and postural compensation may now be visible on imaging. Sleep quality, mood and productivity are often affected. Care at this stage usually takes longer and focuses on function and day-to-day comfort.

Stage 4Critical

Degenerative Cascade

Long-term spinal dysfunction can speed up disc degeneration, bone spur (osteophyte) formation and spinal stenosis. These structural changes cannot be undone. Care at this stage focuses on comfort and everyday movement, alongside your doctor's advice.

It is worth getting checked early.

The longer a mechanical problem is left, the more the rest of the spine tends to compensate around it. An assessment at the acute or subacute stage gives you a clearer picture sooner.
